I understand that I am engaging the Health Care Advocate and Health Coaching services of Guide2Care, LLC  on my own or on behalf of my loved one to obtain information and guidance about organizing and addressing care needs.

 

  • As a client, I acknowledge that the information on this website, blog, services, and products is made available to you as information and education for your use. The information is based on our prior professional experience and background in social services, Healthcare, and other areas. 

  • As a client, I acknowledge that Health Advocate/Health Coach will conduct a need/health assessment to understand the scope of the issues, including but not limited to medical, health, social, or financial.  

  • As a client, I acknowledge that the results are not guaranteed when applying for government services, requesting an increase in-home care hours, requesting an appeal of the decision in-home care hours, and assisting with medical and equipment claims.

  • As a client, I acknowledge that we serve as advocates and guides to help you and/or your loved one to achieve a sustainable quality of life with support and guidance through the use of government and personal funding. 

  • As a client, I acknowledge that I am taking full responsibility for decision-making when seeking advice on health and other services for me or on behalf of my loved one.

  • As a client, I acknowledge that any advice received on health and wellness is not intended to be a substitute for professional medical advice, diagnosis, and/or treatment. We want to improve your relationship with your medical team.  

  • As a client, I understand that Guide2Care LLC will keep notes when interactions are made with you, family members, and any other providers involved in your care and need their assistance to address your specific need.

  • All personal information and medical records shared with the Health Care Advocate of Guide2Care, LLC will be kept strictly confidential unless you, as a client, signed authorization to release information or where disclosures are required by law. 

  • As a client, I agree to hold Guide2Care, LLC, health care advocates, and health coaches, harmless of claims or damages with our work together. 

  • As Health Care Advocates, we agree to provide services respectfully and timely. 

  • As Health Coaches, we are not medical health practitioners or mental health providers. We are not holding ourselves out to be in any capacity. 

  • We provide information to improve your understanding of government services available to you, the best way to utilize them, and help with the application process on a need basis. 

  • We provide information to improve your understanding of your health, and food intake, and help to prepare for your medical appointments.

This privacy policy has been compiled to better serve those concerned with how their ‘Personally Identifiable Information (PII) is being used online. As described in US privacy law and information security, PII is information that can be used on its own or with other information to identify, contact, or locate a single person or to identify an individual in context. We do not use vulnerability scanning and/or scanning to PCI standards. An external PCI-compliant payment gateway handles all CC transactions. We do not use Malware Scanning.

It is only accessible by a limited number of persons who have special access rights to such systems and must keep the information confidential. In addition, all sensitive/credit information you supply is encrypted via Secure Socket Layer (SSL) technology.

All transactions are processed through a gateway provider and are not stored or processed on our servers.
